This final course will focus on the domains of Clinical Practice, Collaboration and Partnership, and Professionalism. Topics will include understanding community programs, data sharing, care agencies, and case management and care coordination. This final course will include a capstone project, teaching nurses how to leverage technology to coordinate care and navigate across the system.
