This course builds on primary care nursing competencies, emphasizing evidence-informed approaches and best-practice clinical guidelines when managing chronic disease and multimorbidity. Learners will address conditions such as mental health and substance use, smoking cessation, diabetes, cardiovascular disease, wound care, and rehabilitation. The course focuses on collaborative care planning and coordination with clients and the healthcare team when delivering and evaluating individualized, culturally safe education and chronic disease management.
